Health Pilot
Description

mPallCare is a digital health platform designed to strengthen palliative care delivery in Uganda by connecting patients, caregivers, and health professionals through real-time data sharing, symptom tracking & management and providing them with decision support tools. The platform enables early identification of patients with life-limiting illnesses and streamlines referral and follow-up care across facilities. By integrating mobile and web-based systems, mPallCare enhances access, continuity, and quality of care for vulnerable patients across rural and urban communities.

Impact

mPallCare improves the quality of life for patients with serious and life-limiting conditions by reducing hospital visits, improving symptom management, and strengthening coordination between healthcare teams. The platform empowers health workers with digital tools for data-driven decision-making and monitoring their patients remotely, reduces costs associated with long-term care, and enhances community-based service delivery. Socially, it bridges urban–rural health disparities, while economically,mPallCare is completely free of charge, it saves time and resources for both families and health systems contributing to universal health coverage and a healthier, more productive population.

Target Audience

The primary beneficiaries include patients with life-limiting illnesses, their caregivers, and palliative care professionals across Uganda. Additional beneficiaries include hospitals, hospices, community health facilities, and policymakers who rely on real-time data for planning and improving healthcare outcomes. The platform also benefits researchers and partners seeking to strengthen palliative and chronic disease management systems.
